VA Form 21-0538 confirms whether the spouse or children on your VA award still have the same dependent status. If nothing has changed, you still need to answer the request: check No in Item 8, then sign and date Section V.

Get the official January 2024 PDF from VA's Form 21-0538 page. The current title is Mandatory Verification of Dependents. You may also see people call it the status of dependents questionnaire.
I checked both pages on October 2, 2026. The footer prints JAN 2024, replacing February 2021. The OMB expiration date, January 31, 2027, is separate from that edition date.
At a glance:
- Purpose: Confirm continued eligibility for additional compensation for a spouse or children.
- No changes: Complete identification, answer No in Item 8, and sign Section V.
- Changes: Answer Yes and complete the applicable spouse or child section.
- New dependents: Use the additional forms identified below rather than trying to add them in a removal field.
- Deadline: Follow the return date in your VA letter. The blank PDF does not print a universal response period.
Who files, and why does VA ask?
VA's manage-dependents page says you must confirm dependent status every eight years. It recommends checking every year and warns that failure to submit the verification by the deadline can cause VA to remove dependents from your benefits.
The same guidance explains why prompt updates matter: if VA continues paying additional benefits after eligibility changes, an overpayment can result. A payment continuing to arrive does not confirm that VA has processed a divorce, death or another change.
Start with the letter that came with the form. Compare its listed dependents with your current circumstances and note the return deadline. Do not assume the letter's list is complete or that a change reported to another agency updated your VA benefits.
For disability compensation, VA generally pays additional dependent amounts when the combined disability rating is at least 30%. This verification concerns an existing dependent allowance. It is not a new disability claim or a request to increase a medical condition's rating.
Section I: identification, Items 1 through 7
Use the veteran's information in this section, even if a family member is helping prepare the response.
- Item 1: First name, middle initial and last name.
- Item 2: Social Security number.
- Item 3: VA file number, if applicable.
- Item 4: Birth date in month/day/year format.
- Item 5: Current mailing address, including unit, country and postal code where needed.
- Item 6: Telephone number with area code, or the international field where applicable.
- Item 7: Email address and a separate checkbox agreeing to electronic correspondence about the claim.
For handwritten responses, the PDF instructs you to print neatly in ink, put one letter in each box and completely fill the applicable checkboxes. Leave the VA date-stamp area unused.
Page 2 also has a field for the veteran's Social Security number at the top. Complete it so the second page remains identifiable if separated from the first. Do not substitute a child's number there.
Section II: status certification, Item 8
Item 8 asks whether your dependents' status has changed. It is the branch point for the rest of the form.
If No: Go to Section V, sign and date, and disregard the intervening change sections. You do not need to fill every child row just to repeat an unchanged list.
If Yes: Complete the section for the dependent whose status changed, then complete the signature section. Several changes may require both the spouse section and one or more child entries.
The note under Item 8 addresses dependents missing from the attached letter. It directs you to 21-686c to add dependents and 21-674 when claiming a qualifying schoolchild aged 18 to under 23. Claim Raven's 21-686c guide explains additions and removals; the 21-674 guide covers school attendance.
Answer Item 8 from the actual status of the listed dependents, and handle an omitted dependent through the appropriate additional form. A new child does not belong in a row whose choices are death, marriage, adoption out or a stepchild leaving the household.
Section III: spouse changes, Item 9
Item 9 has separate choices for a marriage ending and a spouse's death.
If the marriage ended, enter the ending date and select the applicable reason: annulment, divorce or declared void. Use the date supported by the relevant record, not the date you began living separately or completed this questionnaire.
If your spouse died, select Death and enter the date of death. Do not also choose divorce just because you are no longer married.
The section has no field for a new spouse's complete identification or marriage history. A new marriage belongs on the appropriate dependency application. If VA's award still includes a former spouse, make sure that removal is addressed as well as any new addition.
Section IV: child changes, Items 10 through 13
The four child entries follow the same structure. 10A, 11A, 12A and 13A ask for the child's name. The corresponding B field asks how that child's status changed.
Each entry provides these choices:
- Death of child: Enter the date of death.
- Marriage of child: Enter the marriage date.
- Adoption out of family: Enter the date the adoption was finalized.
- Stepchild no longer a household member: Enter the last date the stepchild was a member of the household, subject to the printed exceptions.
Those exceptions matter. The form says not to check the stepchild box merely because you live apart if you still provide at least half the child's support. It also says not to check it when the separation is because of medical reasons, incarceration, school attendance or a military service obligation involving you or the child.
Living at different addresses therefore does not automatically answer the household question. Read the note beside the exact box before selecting it. If you cannot tell whether the facts fit, ask VA or an accredited representative to help interpret the situation.
Item 10 begins on page 1. Items 11 through 13 continue on page 2. Use one entry for each affected child. The PDF says to use a separate 21-0538 if more than four children's statuses have changed.
A schoolchild stopping attendance is not one of the four change choices printed here. Use VA's dependency guidance and the relevant school or dependency form rather than checking an unrelated box. The 21-686c has a section specifically for a schoolchild over 18 who stopped attending school.
Section V: sign and date, Items 14A and 14B
In 14A, the veteran signs the certification that the information is true and correct to the best of their knowledge and belief. 14B is the actual signing date, in month/day/year format.
This is required even when Item 8 says No. The shortened no-change path saves you the change sections, not the signature.
Before signing, compare every event date with your records. Keep the questionnaire, attached VA letter, any additional forms and your submission confirmation together. They make it easier to explain what you reported if a later notice uses different facts.
Fictional example: a divorce with no child changes
This is a fictional worksheet exercise, not a real household or claim. No personal identifiers are provided.
Assume VA's letter lists a spouse and a biological child. The veteran's divorce became final on June 12, 2026. The child's dependent status has not changed.
- Section I: The veteran completes their own identification and current contact information.
- Item 8: Yes, because the listed spouse's status changed.
- Item 9: Marriage ended, Divorce, with 06/12/2026 as the ending date.
- Items 10 through 13: No child-change entries for the unchanged biological child.
- Items 14A and 14B: The veteran signs and dates the response.
For comparison, if both the marriage and child's status were unchanged, the veteran would select No in Item 8 and go directly to Section V. These are two different fact patterns, not alternative answers to choose based on which would preserve a payment.
Where to submit VA Form 21-0538
Mail: The January 2024 PDF directs the completed form to Department of Veterans Affairs, Evidence Intake Center, PO Box 4444, Janesville, WI 53547-4444.
Online: Start with VA's manage-dependents page for its confirmation options. For a completed PDF, VA's QuickSubmit guidance describes uploading documents to the Evidence Intake Center through AccessVA. Retain the upload confirmation; opening or typing in a PDF alone does not submit it.
Fax for compensation correspondence: VA's compensation intake page lists 844-531-7818 domestically and 248-524-4260 for foreign claimants. These are compensation intake numbers, not a separate fax line printed on 21-0538. Follow any specific return instructions in the verification letter.
I verified the public submission guidance, not a signed-in completion of VA's online dependent-verification workflow. The paper form and published intake routes remain available if the online option does not load.
Check the result after returning the form
Watch for VA correspondence and answer requests for clarification. Review the dependent information and any payment adjustment VA reports. If you receive a debt notice, read its explanation, response options and deadlines; sending a dependency form does not by itself settle a dispute about an existing debt.
