VA Form 21-0788: Information Regarding Apportionment of Beneficiary's Award

What VA Form 21-0788 is for

An apportionment pays part of a beneficiary's VA award to another eligible person. The spouse, child or dependent parent asking for it fills out this form, or someone acting for a minor child. The February 2026 form asks for one of a short list of qualifying reasons, such as the veteran being incarcerated for more than 60 days after a conviction.

What you fill in by hand

The filler never asks for your Social Security number, VA file number, date of birth or signature. You write those in by hand. Leave these boxes for yourself:

• Item 2: the veteran's Social Security number
• Item 3: the veteran's VA file number
• Item 4: the veteran's date of birth
• Item 10B: each person's Social Security number
• Items 15A and 15B: signature and date

• In person: you can also hand it in at a VA regional office.
• Keep the signed form, your attachments and your receipt together. The date VA receives the claim matters for which rules apply.
