What VA Form 21-674 is for

VA uses this form to decide benefits for a veteran's child who is at least 18 but under 23 and attending school. Use a separate form for each student. Tell VA before your child turns 18 if school attendance will continue.

Never received additional benefits for this child? The 21-686c instructions say to send Form 21-686c with this one. To report that the child left school or married, VA points to Form 21-674b. Fill out VA Form 21-686c.

What you fill in by hand

The filler never asks for your Social Security number, VA file number, date of birth or signature. You write those in by hand. Leave these boxes for yourself:

• Item 2: VA file number
• Items 5 and 6: the student's Social Security number and date of birth
• Item 16D: international phone number
• Items 16A and 16B: signature and date

• Online: start VA's combined 21-686c and 21-674 application on VA.gov and pick the option for adding a child age 18 to 23 who will attend school.
• Disability compensation, by mail: Department of Veterans Affairs, Evidence Intake Center, PO Box 4444, Janesville, WI 53547-4444.
• Pension or survivors benefits, by mail: Department of Veterans Affairs, Pension Intake Center, PO Box 5365, Janesville, WI 53547-5365.
• If a VA letter gives its own return instructions, follow them.
• Keep a copy of the signed form and your submission receipt.
