Asking for a form and filing a complete claim are different steps. Other rules determine what counts as a claim, how an intent to file works, and which deadlines or effective-date protections apply.

Keep the form request separate from the application
A request for an application is part of getting the process started, but it may not contain the information needed to decide a benefit. Save the response from VA and check that the form provided matches what you are trying to apply for.
If a family member is dealing with a veteran's death, several different benefits or pending matters may be involved. The appropriate application depends on the benefit and the applicant's relationship. A letter about one benefit should not be assumed to cover every survivor-related matter.
Does requesting a form preserve a payment date?
Do not assume that from this section alone. Rules about an intent to file, a complete application and effective dates address that question. Keep the request date, any intent-to-file confirmation and the completed application's receipt so the actual filing history can be checked.

§ 3.150 Forms to be furnished.
(a) Upon request made in person or in writing by any person applying for benefits under the laws administered by the Department of Veterans Affairs, the appropriate application form will be furnished.
(Authority: 38 U.S.C. 5102)
(b) Upon receipt of notice of death of a veteran, the appropriate application form will be forwarded for execution by or on behalf of any dependent who has apparent entitlement to pension, compensation, or dependency and indemnity compensation. If it is not indicated that any person would be entitled to such benefits, but there is payable an accrued benefit not paid during the veteran's lifetime, the appropriate application form will be forwarded to the preferred dependent. Notice of the time limit will be included in letters forwarding applications for benefits.
