What this means

This appendix points pension Veterans Service Representatives (VSRs) to their task-based quality review materials and lists special issues and corporate flashes used in system-compliance review. These labels help identify the type of work or handling recorded in VA's systems. A listed label is not, by itself, evidence that a person qualifies for a benefit or has committed wrongdoing.

How this may help with a claim

Use the two lists to understand a special-issue or flash entry mentioned in your records. Check which list applies and whether the relevant entry was recorded correctly. Missing entries outside these lists are not automatically ignored: the appendix says required unlisted entries are noted as remarks and returned for correction.

What to review in your file

  • The actual VSR checklist, instructions and review elements are not reproduced here. Their three destinations are M21-4, Chapter 6, Appendix A.a, A.b and A.c, respectively. Read the appropriate destination before treating this appendix's lists as the whole review standard.

  • Use the special-issue table for its exact entries. It includes section 1151, two levels of administrative and rating review, the separate Agent Orange and Camp Lejeune labels, ALS, C-123, character of discharge, chemical/biological issues, duplicate records, JSRRC requests, local hearing, quality and mentor review, medical foster home, mustard gas, national quality review, radiation and confirmed radiogenic disability, RVSR examination and specialized records requests. Do not merge distinct system labels into one finding.

  • Corporate flashes have a separate list. It includes section 1151, ALS, Board decisions, blindness, C-123, clear and unmistakable error, congressional interest, death in service, fraud-denial and fraud-termination labels, disappearance, FBI and foreign cases, fugitive felon, homelessness, incarceration, OIG and Central Office investigation, POW, potential fees, potential danger, private-attorney fees payable or not payable, Purple Heart, Secret Service and terminal illness. These are system categories, not new definitions of those conditions.

  • For both special issues and corporate flashes, the source distinguishes listed system-compliance errors from other required entries. An unlisted entry that still needs to be added is identified as a remark and returned for correction. Being absent from these tables is not permission to omit required information.

Important limits

The lists are source-dated March 24, 2025. They do not establish current system availability, benefit eligibility or the facts behind a sensitive label. The detailed checklist and review elements remain in the cited Chapter 6 appendices; no missing rules from those destinations have been inferred.
