What this means
The FSR IQR sample reviews fiduciary decisions and the actions that follow them. It distinguishes a competency decision, closing a field examination, protecting funds and processing payments from later review, accounting and misuse work.
How this may help with a claim
Use the headings to identify which part of a record needs an explanation. A concern about a withheld payment is different from a concern about a fiduciary appointment, a notice or a misuse determination. The sample helps keep those questions separate.
What to review in your file
For the decision and examination closure, look for the stated basis, due-process record, required notice and documentation of the fiduciary's role. The sample also checks follow-up arrangements and referrals to the office responsible for additional work. These are distinct review points, not proof that a decision was correct.
Trace protection and payment actions separately. The checklist includes bonds, properly titled accounts, withheld or released retroactive funds and the records supporting those actions. It then examines award actions outside the EP 590 examination process and particular benefit adjustments. Listing an adjustment does not establish that it applies to a person's award.
Check the record for the particular review or financial task. The sample addresses review templates, hearings or conferences, the decision's evidentiary support, accounting audits and misuse documentation. It also checks notices, referrals, debt-related actions and system controls. Use the governing procedure to determine what evidence a review may consider.
Important limits
This is an internal sample, not a new rule for decisions or review options. In question 6, an error-description row says that an HLR was not processed with additional or newly submitted evidence. That wording is not treated here as permission or a requirement to add evidence to an HLR. The separate FSR Review Guide PDF is inaccessible; G.b also points to F.a rather than the displayed G.a checklist. Those source limits remain unresolved.
