What this means

A special issue is an indicator attached to a particular VA claim. It may identify the claim type, a disability or disease, or another circumstance relevant to that claim. The section tells staff to identify and enter applicable special issues, and notes that one claimed disability or set of symptoms can warrant more than one.

The current index pairs a name and code with its description; the historical index separately records discontinued names, codes, dates and former uses where supplied. Some entries describe an alleged exposure or condition, others route work to a specialist or reviewer, and others identify automation, evidence or payment-review work. A special issue therefore describes how work is identified or handled, not a benefit decision or proof that every condition in the underlying program has been met.

How this may help with a claim

When an unfamiliar special issue appears in a record, match both its name and code to the relevant description and ask what claim-specific fact or pending action it represents. For example, a private Disability Benefits Questionnaire entry is different from a VA entry for a questionnaire completed by a VHA clinician outside the compensation-examination process. Identifying the correct category can focus a question about whether the evidence or required review was recognized; it does not establish the weight VA must give that evidence.

A routing label can also help explain a handoff. Records Research Request calls for review by a regional-office research coordinator and is removed after that review results in either submission of a research request or verification of the claimed exposure or stressor. Its removal alone therefore does not tell you which outcome occurred. Ask for the resulting action or record rather than assuming that removal means the exposure was disproved or the claim was decided.

What to review in your file

  • Read the exact name, code and description together, and check all applicable claim circumstances. More than one special issue may apply to a contention. Some rows have no description, and a code alone cannot supply the missing instruction. The two Foreign Out-of-Network rows for AME and SSSCE both show FOONAME despite describing different examination processes; do not choose between them by guessing that one code is a typo. The Nehmer Legacy Appeal Review description also ends with an incomplete instruction to add something to the claim. Follow its referenced guidance instead of reconstructing the missing words as an operational rule.

  • Separate a claim's alleged condition or exposure from an established entitlement or a completed evidentiary finding. Many disease and exposure rows expressly apply when entitlement has not previously been established; the precise condition differs by row. The PTSD entries distinguish combat, non-combat and non-sexual personal trauma, while the MST entry covers mental or physical conditions, including PTSD, resulting from MST. The non-PTSD personal-trauma entry is limited to non-sexual trauma and excludes PTSD. For questionnaires, the Private entry covers a DBQ from a non-VA source; the VA entry covers one completed by a VHA clinician outside the C&P examination process. These are classification instructions, not substitute diagnoses, stressor findings or rules for weighing medical evidence.

  • For a routing or review issue, check who acts, what action is pending and when removal is authorized. Administrative Decision Review and Rating Decision Review Level 1 route a draft to a second signer and are removed on finalization or routing to a third signer; Level 2 routes to the third signer and is removed on finalization. Local Quality Review remains during an individual-quality-review rebuttal and is removed only when no correction is needed or correction is confirmed. Specialized Records Request is removed when the military-records-specialist request is submitted, whereas Records Research Request follows the coordinator's completed review and submission or verification outcome. Pension & Fiduciary Service Review is limited to designated pension-management-center personnel. These internal routing conditions are not a new claimant appeal procedure or a deadline for a benefit decision.

  • Do not assume that all automation or exposure-review labels can be removed when a task ends. Automated Claim — APC applies only to automation-eligible contentions in a mixed claim containing at least one ineligible condition. Automation Claim Review, its ADS variant and Rapid Ready for Decision have their own eligibility or pilot/prototype scopes, and the source tells claims processors not to remove those listed issues. RBPS Re-Run Inventory describes a different system-managed removal when a claim leaves that automation process. The PACT entry lists alternative qualifying claim circumstances and says not to remove it after a TERA examination merely because the opinion is negative or attributes the condition to another cause. The separate PACT ACT DIC Reevaluation entry requires a valid elected reevaluation claim under its cross-reference and prohibits applying it and PACT to the same EP simultaneously. None of these tracking instructions determines the outcome of the underlying claim.

  • Treat the FDC exclusion rows as a lookup requiring verification, not a self-contained set of program rules. Several names and descriptions do not line up: Additional Claim Submitted describes additional evidence, while Evidence Received After FDC CEST describes an additional claim. Necessary Form(s) not Submitted also describes additional evidence, and Needs Non-Fed Evidence Development describes required forms not submitted together. Do not silently swap those descriptions or tell a claimant which label applies on that basis alone. Consult the referenced Fully Developed Claim procedure and actual submission history before interpreting an exclusion. A fully developed-claim label or exclusion label is not itself an approval or denial of benefits.

  • Check the historical index and any date condition before treating an old label as available today. The separate historical table includes retired appeal, pilot, exposure, attorney-fee, examination and processing labels; its discontinuation dates concern those indicators, not automatic termination of the associated benefits. Current-table entries can also have historical context, including RAMP review-error descriptions and a Character of Discharge entry for determinations before June 25, 2024, alongside COD AQ95 for determinations on or after that date. Blank descriptions, blank historical dates and old program names do not justify inventing a current instruction. Use the source's date and referenced procedure to distinguish an archived label from an active action in the particular record.

Important limits

This recovered topic is dated September 4, 2026. It explains the purpose of claim-specific indicators and selected important distinctions across the current and historical indexes; it does not reproduce the full substantive requirements of every referenced program or certify every code for present-day system use. The many undefined entries, internal project labels and external user-guide references limit what can safely be inferred. The duplicated foreign-examination code, mismatched FDC descriptions and incomplete Nehmer instruction remain source issues, not editorially corrected facts.

A special issue can mark an allegation, an evidence category, a requested review or an automated process without establishing eligibility, a medical conclusion, payment entitlement or a completed action. Internal routing and review-time instructions should not be recast as claimant filing deadlines or promised decision dates. This newly recovered source and its explanation require their own review; they do not inherit approval from an older catalogue or prove that any existing record was correctly classified.

Special issues are claim -specific indicators and can represent a certain claim type, disability or disease, or other special notation that is only relevant to a particular claim. Special issues must be identified and inputted when applicable. If a special issue exists and applies to the claim, it is required.

Note: Scenarios may exist where the contention (claimed disability or signs and symptoms of a disability) would warrant multiple special issues.
