QA Purpose

M21-4 section 8.1. Official source text with a separately reviewed Claim Raven explanation when available.

Claim Raven wrote the explanation that follows. The original VA text appears below it, unchanged.

What this means

m21-4:8.1 explains QA purpose. In plain terms, the official guidance says quality Assurance (QA) is the Fiduciary Program’s national process for measuring fiduciary activity processing accuracy. This includes field examinations, accountings, and Fiduciary Contact Center (FCC) activity. It also addresses QA results are generated and published for all fiduciary hubs (hubs) and the FCC. The results are included in the hub’s, FCC’s, and Regional Office’s (RO’s) Director’s Performance Dashboards.

How this may help with a claim

Use m21-4:8.1 as an internal VA audit trail for QA purpose, not as a promise of a particular result. Compare the actions recorded in your claim file or decision notice with this source-specific detail: Quality Assurance (QA) is the Fiduciary Program’s national process for measuring fiduciary activity processing accuracy. QA results are generated and published for all fiduciary hubs (hubs) and the FCC. The results are included in the hub’s, FCC’s, and Regional Office’s (RO’s) Director’s Performance Dashboards. If the record does not show the expected action, identify the missing step precisely when asking VA or an accredited representative to review the file.

What to review in your file

  • Check the file against this official condition: methods to determine and improve quality, and
  • Confirm that the record or notice addresses this source point: 8.1.b. Methods to Determine and Improve Quality
  • Document how this stated step or exception applies: Pension and Fiduciary (P&F) Service requires effective quality reviews and positive action to improve quality levels for all fiduciary work.

Important limits

m21-4:8.1 is primarily internal workflow guidance about QA purpose. It can help identify what VA was expected to document, but it does not by itself create a claimant deadline, a freestanding entitlement, or a guaranteed remedy. The source also states: Quality Assurance (QA) is the Fiduciary Program’s national process for measuring fiduciary activity processing accuracy. P&F Service QA results assist in monitoring the level of service to beneficiaries in the Fiduciary Program.

8.1.a. QA Purpose

Quality Assurance (QA) is the Fiduciary Program’s national process for measuring fiduciary activity processing accuracy. This includes field examinations, accountings, and Fiduciary Contact Center (FCC) activity.

QA results are generated and published for all fiduciary hubs (hubs) and the FCC. The results are included in the hub’s, FCC’s, and Regional Office’s (RO’s) Director’s Performance Dashboards.

8.1.b. Methods to Determine and Improve Quality

Pension and Fiduciary (P&F) Service requires effective quality reviews and positive action to improve quality levels for all fiduciary work. The following subchapter describes methods used to determine quality levels and improve quality on an organized technical basis. The methods may consist of

  • regular supervision and training
  • mandatory or optional reviews and spot checks
  • controls such as the QA program, and
  • special focused quality improvement reviews.

8.1.c. Hub and FCC Responsibilities

P&F Service QA results assist in monitoring the level of service to beneficiaries in the Fiduciary Program. QA performs quality observations and reviews in all areas of hub and FCC operations on a continuing basis.

The quality review system does not require that evaluations encompass every work team within the hubs.
