Save symptom entries and return from a task
Record dated symptoms, reopen the exact saved entry from Action Plan and keep different condition logs separate.
Overview
Keep dated notes about symptoms, then return to the same saved entry from Action Plan. Each entry keeps the condition, date, severity and details you entered.
Start from a task
Choose the symptom log Symptom Tracker condition picker with search, Condition log selection, and Log, History and Summary navigation. Choose a condition before writing an entry. This image shows the picker, not a completed save or task return. Search by condition name or an alternate name. History is where you return to saved entries. Open a task whose next step is Track symptoms. Check the condition and workspace shown above the log. Choose One day to record one date. Enter the severity, answer the prompts that apply and add details about what happened or what it stopped you doing. Select Save entry. Several days lets you select dates and review a separate entry for each day. Describe only what you reliably remember; do not copy one day's experience into days you cannot recall.
Return to the saved entry
Wait for Saved entry linked to your task. Select Back to Action Plan, then use Open saved symptom entry. It opens History with the saved row marked Selected entry. Saving does not complete the task or change its notes, due date or follow-up date. A later deliberate entry can become the current task link; earlier links remain in its source history.
Finish an interrupted save
Use Retry saving for pending entries. This retries the same entries. If browser storage is unavailable, keep the page open until saving finishes. If the entry saved but the task link did not, find that entry in History and use Link to task. A task for one condition cannot link an entry for a different condition.
Assign an existing log
In the standalone tracker, use Add an existing symptom log to a claim. Choose the condition log and workspace, then Save. Only that condition's entries that are not already assigned move into the workspace. Open the workspace's Drafts & notes to find its separate condition logs. Older browser entries with no verified account are left on the browser; they are not automatically imported into the current account.
If you cannot continue
If you cannot continue An entry saved but did not return from its task Look for the entry in History. Check that its condition matches the task. Use Link to task for an already-saved matching entry. Use Retry saving for pending entries, and keep the page open if browser storage is unavailable. For product help, describe the page, the message shown and what you were trying to do. Keep private records and account details out of screenshots.
