Organize evidence without renaming everything

Keep original files unchanged, use a simple index for source, dates, contents, and questions, and create final submission copies only when needed for a verified official route.

Overview

You do not need to rename, rewrite, split, or combine every record before you can understand it. Preserve each original, make a working copy only when needed, and build a short index that says what the item is, where it came from, what dates it covers, and which question it may address. The index points to evidence; it is not the evidence An index, Pathfinder record list, AI extraction, or evidence task cannot replace the original document, determine legal sufficiency, confirm a filing, or prove that VA received a file.

Aim for traceability, not a perfect filing cabinet

What is it? Identify the treatment note, imaging report, service record, statement, decision, form, receipt, or other record type. Where did it come from? Record the VA source, private provider, military custodian, witness, or veteran. What question may it address? Describe current condition, service fact, medical relationship, severity, timing, process, or a notice-specific request without claiming the item proves the issue. Can you reproduce it? Preserve the original and document the exact final copy and receipt if submitted.

Preserve originals before making changes

Treat the file received from the source as the original. Keep it unchanged. If you need to rotate pages, add bookmarks, redact an unrelated person's information, split a large file, or assemble selected pages, work from a copy. Avoid accidentally deleting a page while editing a PDF. Preserve the provider's filename, metadata, and download date. Keep a redacted support-safe copy separate from the complete medical record. Preserve which pages came from which source. Keep the original reproducible even when the filename is not descriptive.

Use a small folder structure

01-original-records 02-working-copies 03-statements-and-forms 04-final-submissions 05-receipts-and-va-notices Group originals by source, year, or broad type only when useful. A structure you maintain is better than a complex one you abandon. Keep final submission packets separate from the source archive: one answers what did I send, while the other answers what do I have.

Build the evidence index

Short item ID and neutral title. Source, author, record date or range, and date received. Page count and private archive location. Condition, issue, or question the record may address. Original, working-copy, or final-submission status. Destination, submission date, exact filename, and receipt reference when submitted. Describe contents without deciding the claim Write documents a diagnosis, describes an event, records measurements, or reports missed work observed by a supervisor. Do not write proves service connection or sufficient evidence unless the source itself and a competent decision support that statement.

Connect records to questions

Organization labels are not legal findings. Label Question it organizes CURRENT Current diagnosis, symptoms, testing, treatment, or impairment. SERVICE Event, injury, disease, exposure, duty, or service period. LINK Medical reasoning or other evidence addressing a relationship. SEVERITY Frequency, duration, measurements, treatment, or functional limits. TIMING Onset, progression, effective-date fact, or response date. PROCESS Receipt, notice, examination, authorization, or decision. One record can relate to more than one question, and a label never means the evidence is complete. Check the actual claim type and source before collecting or submitting anything.

Handle large medical files

Keep the complete export as the original. Add one index entry for the full file and, when useful, a page map for relevant sections. Synthetic page map for a large source file. A smaller submission copy should remain traceable to the complete original.

Control duplicates

Compare the records Check source, date, provider, page count, contents, and whether one is a corrected copy or overlapping export. Choose the authoritative original Preserve one source copy and record its relationship to duplicates or later versions. Keep submitted versions Do not delete a final submitted copy because the source original also exists. Avoid guessing The same visit can appear in several exports without being an exact duplicate. Record the overlap.

Prepare a submission copy deliberately

Confirm every page is readable, correctly oriented, and belongs to the veteran. Preserve visible dates and provider information when relevant. Remove unrelated private information only from a working copy and keep the original. Open the exported file and verify the neutral filename. Use the current official route for the correct pending claim, review, appeal, or other task. Save the exact final file and receipt. Recheck technical limits before upload VA's accepted file types, size limits, displayed document coverage, and submission routes can change. Use the current official page rather than an old checklist or screenshot. Decision-review and appeal evidence use their own current instructions, including QuickSubmit where VA directs it. A file picker that accepts a document is not proof that the route fits the matter.

Keep submission history separate

The private log preserves what was sent even when the status tool is incomplete. Log field What to keep When Date, time, and time zone. Where Destination, claim or process, and route. What Exact filenames, page counts, and submitted versions. Proof Confirmation or tracking number and a copy of the receipt. VA's claim-status tool may not display every document sent by mail, fax, in person, or under privacy restrictions. Your source archive, exact submission copy, and official receipt remain essential.

Use records in Pathfinder

Review source records in Records Check-In Confirm file identity, type, processing status, coverage, and the original source before relying on an extraction. The record remains traceable without claiming full archive coverage or VA receipt. Link a reviewed record in Evidence Builder Connect the record to one condition-scoped question only after checking the relevant page and source. The link organizes work but does not prove the task is satisfied or the evidence sufficient. Preserve private originals and official proof Keep the archive and final submissions outside Pathfinder in a private location you control. Pathfinder status remains separate from the authoritative source file and submission receipt.

Common mistakes to avoid

Renaming or editing the only source copy. Combining unrelated records into one unindexed file. Uploading an entire archive because some pages might be relevant. Deleting duplicates before confirming which version was submitted. Treating an evidence table or AI summary as the underlying proof. Writing conclusions the record does not state. Using sensitive identifiers in filenames. Losing the exact final packet and receipt. Assuming Pathfinder proves VA receipt. Following old technical upload limits.

When the evidence archive is messy

When the evidence archive is messy Many files have generic names. Leave originals unchanged. Open each source and record a neutral index title. Add source, date range, and page count to the index. Use the index for findability instead of renaming every original. Two files contain overlapping records. Compare source, dates, page count, and content. Identify corrected or later versions. Record overlap and submitted status. Keep an authoritative original and preserve any final submitted copy without guessing that the files are exact duplicates. A large export contains a few relevant pages. Preserve the full original. Create a page map. Keep complete pages and surrounding context in any working copy. Make a traceable submission copy only when the specific task and route are confirmed. The status tool does not show my document. Check the route used and original receipt. Remember that some mail, fax, in-person, or restricted documents are not displayed. Avoid automatic duplicate submission. Use official proof and the current VA contact route rather than treating the Pathfinder or online list as conclusive. Return to Records Check-In or Evidence Builder to correct source and task metadata. Keep original records and official receipts in the private archive you control.
